Data Subject Request Form – Policy and Procedure

The Gardner Institute respects and believes in the rights of users to obtain a record of all personal data held by Gardner Institute. Submission of the following form constitutes both a legal and ethical charge to Gardner Institute to generate a record of all personal data held by the submitter based on information provided, free of charge and in a timely manner.

Instead of or in addition to this form you may email data@jngi.org with a record request or information request about Gardner Institute’s data protection policies and procedures. After a request has been submitted you may be contacted by a Gardner Institute staff member to verify the authenticity of the request.

 The Gardner Institute reserves the right to request additional information to verify the identity of the requester.
